Things to condider when choosing a cake:
Flavor - chocolate, vanilla, or a combination and carrot cake are the most popular
Filling - same as frosting, or a contrast; raspberry or apricot jam adds a nice fruity contrast
Frosting - basic white buttercream is the most popular for wedding cakes
Shape - round, square, heart-shaped, etc.
Configuration - tiered, with pillars, separate, sheet cakes, etc.
Number of guests - this will be the basis for size and price of the cake
Topper - bride and groom figurine, fresh flowers, you name it

 Themes
Do you have a theme for your wedding? What are your colors?

A beach theme is very popular, with seashells and blue hues. Chocolate shells, edible pearls, and rope fondant are en example of popular decorations for a seaside theme. Here's and example.

Another idea for an Autumn wedding is a color combination of reds, oranges, and yellows with leaf decorations.
 1st Anniversary Tradition
Decide whether or not you would like to have an extra cake top - be it the actual cake top or boxed on the side - to be saved for the anniversary.

The cake top is a nice tradition, and if used I suggest eating it after the honeymoon (perhaps with some friends?), as it tastes pretty awful after whole year.
 Picture it
It helps if you have a picture that is similar to what you are looking for, or at least have an idea of what you already want. Do you like the traditional look, with lots of different borders and decorations, or a more modern look, with clean lines and simple decorations?

NOTE: rolled fondant is a popular choice for a smooth, modern look, but I won't work with it. I use buttercream, and while it doesn't smooth out to the same degree, it tastes a whole lot better.
